Their website says: Guests with disabilities may be dropped off at the stadium arch in the east parking lot outside Gate D. A state issued Persons with Disabilities license plate or an ADA hang tag placard must be visible to the police and parking attendants and the person to whom the placard or license plate is registered must be an occupant of the vehicle in order to access the Gate D drop off area. Disabled guests with suite or club seat tickets may enter Gate D. All other guests with disabilities may access the stadium by using the ADA ramp located at Gate C in the northeast corner of the stadium. Guests with disabilities may park in the designated disabled parking section in Lot E3, just between Gates C and D. A state issued Persons with Disabilities license plate or an ADA hang tag placard must be visible to the police and parking attendants, and the person to whom the placard or license plate is registered must be an occupant of the vehicle in order to park in the disabled parking section. An accessible entrance to the park is located just north of the disabled parking section at Gate C.
Everything is either flat or there's ramps where it's not
Yes, there are elevated platforms (with ramps) on the floor/field towards the front part of the stadium. You’re allowed to have one companion with you in that area. Their website says: Seating for guests with disabilities is available in all price categories. To purchase seating for guests with disabilities, call the Ticket Office for more information. In order to assist disabled guests, please phone TOYOTA PARK’s main number 708-594-7200 or the Ticket Office at 708-594-7700 prior to arrival.

This is a stadium generally used for soccer, so when it is used for concerts the seating details may change depending on the show and how ticketing is organized.
